This 14-year-old patient was brought to the Charlotte Gynecomastia Center by his parents after he began to experience emotional distress related to the appearance of his chest. He had been made fun of at the pool and teased at school, and his self-esteem had taken a noticeable hit.
At first glance, his chest resembled hanging breasts. But upon evaluation, we confirmed that the nipple position was actually appropriate for his age and body type. The issue was not sagging or excess skin. It was a significant overgrowth of glandular breast tissue combined with localized fat. The chest appeared heavy and disproportionate, but the foundation for a natural chest shape was already there.

Why Early Treatment Made Sense
Although puberty-related gynecomastia often resolves naturally, this case was different. The tissue had become firm and dense, and the patient had already begun to withdraw socially. At 14 years old, he was at a critical point in his physical and emotional development.
We felt confident that early surgical intervention would provide both cosmetic improvement and emotional relief, and that doing so before further body changes would make for a smoother, more natural transition.
Customized Surgical Plan
This was a more advanced case due to the size of the chest, the patient’s weight, and the combination of gland and fat present. After a full consultation and imaging, we created a surgical plan that addressed all layers of the problem while protecting the integrity of the skin.
Procedure Details
- Breast tissue removal: We excised 138 grams of dense glandular tissue through a hidden incision at the base of the areola. This corrected the protruding fullness that was resistant to change.
- VASERlipo: We used ultrasound-assisted liposuction to remove 825cc of fatty tissue from the chest. This helped sculpt the surrounding area and reduce the overall weight and bulk of the chest. VASER was particularly effective in breaking up fat while preserving healthy tissue and minimizing trauma.
- No skin removal: Even though the chest appeared saggy, we preserved all of the patient’s natural skin. We anticipated that with youth, healing capacity, and future growth, the skin would retract on its own. And it did.
- Scar management: The only incision was placed along the lower border of the areola. With time, this scar became almost invisible and did not interfere with his chest’s natural appearance.
- Anesthesia: The procedure was done under general anesthesia, which is often preferred in younger teens for comfort and safety. The patient was discharged the same day.
5-Month Results: Early Signs of Success
At the 5-month mark, the transformation was already dramatic.
Immediate Improvements
- Flattened chest contour with removal of both gland and fat
- Natural nipple position maintained without needing to move or lift the areola
- Skin contraction already in progress, showing smooth transitions across the upper torso
- Improved confidence and posture reported by both the patient and his parents
The chest looked proportional and no longer resembled a breast shape. Best of all, the skin had responded beautifully to the underlying changes without the need for removal or lifting.

5-Year Follow-Up: A Long-Term Success Story
At age 19, the patient returned for follow-up photos. He had grown to 6 feet tall and had lost 35 pounds since surgery. What makes this case so special is the way the surgical results aged with him and improved over time.
Long-Term Outcomes
- No skin redundancy or sagging despite significant growth and weight loss
- Chest looks naturally masculine, firm, and proportional to his new adult frame
- No visible scarring, even with darker skin tone and substantial change in volume
- Results held steady over five years, proving that early treatment was not only effective but also stable long term
It is rare to be able to follow a patient this far beyond surgery, and this case is living proof that the right intervention at the right time can deliver lifelong confidence and balance.
Emotional and Developmental Impact
The difference in his appearance is striking, but the difference in his presence is even more powerful. Before surgery, this patient had begun to pull back from social life and team activities. Now, five years later, he carries himself with comfort and ease.
This was more than cosmetic. It was a reset to help him grow into his adult identity without shame or self-doubt. And it worked.
