This 45-year-old patient came to the Charlotte Gynecomastia Center with concerns about the size and shape of his chest. At 6’2″ and 265 pounds, he had a large, broad frame and carried significant volume in the chest area. His gynecomastia had developed over time due to excess fat combined with firm glandular tissue. He described his chest as “heavy” and noted that it did not match the rest of his body, even as he worked on improving his health.
Like many of our patients, he was clear about what he did not want. He did not want scars across the chest. He did not want major incisions. And he did not want to deal with the recovery or risks of general anesthesia. His goal was a smaller, more contoured chest with skin that would tighten on its own without surgical excision.

Listening to the Patient’s Priorities
This patient wanted a chest that looked flatter and more proportionate to his body. He was not seeking a highly defined or muscular look, but he did want to eliminate the hanging fullness and soft projection that had kept him from feeling comfortable in fitted shirts.
His specific goals included:
- Reducing the visible size of the chest
- Avoiding any large scars or skin excisions
- Staying awake during the procedure
- Tightening the skin without having to remove it surgically
Based on these priorities and his physical exam, we designed a treatment plan focused on advanced liposuction, energy-based skin tightening, and selective gland removal, all performed while awake under local anesthesia.
Awake Gynecomastia Surgery with Skin Tightening
This patient underwent surgery in our in-office accredited facility. He remained awake the entire time, relaxed and pain-free. By using tumescent anesthesia and a gentle surgical approach, we were able to avoid general anesthesia altogether.
His procedure involved three major components: VASERlipo, gland excision, and Renuvion skin tightening.
Step 1: VASERlipo for Fat Removal and Chest Contouring
VASERlipo uses ultrasound energy to break down fat before it is suctioned out. This technology is especially useful in larger patients with dense or fibrous fat, as it allows us to remove volume more smoothly and with less trauma to the surrounding tissue.
What VASERlipo Accomplished in This Case:
- Removed 1400cc of fat (the equivalent of 3.2 pounds) from the chest area
- Contoured the outer edges of the chest wall for a more streamlined profile
- Created a more defined transition between the chest and upper abdomen
VASER also helps preserve skin integrity, which is important when we are relying on the skin to tighten over time.
Step 2: Direct Glandular Tissue Excision
While fat alone can make the chest appear full, true gynecomastia also involves dense breast tissue that sits behind the areola. In this patient’s case, the glandular tissue was substantial and contributed to the round, projected shape of the chest.
We removed this glandular tissue through a small 2-centimeter incision hidden along the lower edge of the areola. No additional incisions were needed, and there were no scars across the chest.
This excision allowed us to flatten the central portion of the chest while avoiding over-resection or contour irregularities.
Step 3: Renuvion Skin Tightening to Avoid Skin Removal
In patients with heavier chests, one of the most common challenges is managing the extra skin that remains after fat and gland removal. This patient did not want to undergo skin excision or accept visible scars across the chest.
How Renuvion Helped:
Renuvion uses radiofrequency energy delivered through a small probe beneath the skin. This energy contracts the collagen matrix and stimulates long-term remodeling. The result is natural skin tightening that continues to improve over several months.
We used Renuvion across the lower chest and lateral chest wall to help the skin retract and conform to the new underlying shape. No skin was removed during this procedure. The only incision was the small areolar entry point used for both the excision and energy treatment.
Two Months Post-Op: Early Progress with More to Come
At just two months after surgery, his results are already showing a dramatic improvement.
What We See at This Stage:
- Significantly flatter chest contour with a clear reduction in volume
- Improved symmetry and shape from the side and front views
- No hanging skin or sagging chest
- Only a small, well-hidden scar along the lower areola
- No signs of overcorrection or cratering
It is important to remember that skin tightening from Renuvion continues for up to six months. That means his results will continue to improve with time as the skin further retracts and adapts to the new shape.

A Personalized, Non-Invasive Approach
This case highlights what is possible with modern gynecomastia techniques that prioritize patient comfort and aesthetic outcomes. By combining:
- Awake VASERlipo
- Targeted glandular excision
- Advanced radiofrequency skin tightening
We were able to achieve a powerful transformation without the need for large scars or major surgery.
